Arizona's Public Safety Incident Notification System!
You can signup with ICP in two ways:
You can download our form and fill it out offline or you can fill out the form online and then print it out and mail it in. Both of these forms are in Adobe Acrobat format and you can get the free Adobe Reader here if you don't have it already.
Download our offline signup form by right clicking here and then use the "save target as" to save the "CurrentApplication.pdf" to your hard drive.
Start by filling out and signing all pages of the forms. Then, mail the forms, and a check for your annual membership fee (payable to "Incident Command Page") to the address listed on the form.
To fill out the form online and print it out on your printer to mail in click here.
Within 24 hrs a representative of ICP will contact you.
